Corrective Actions: Manager to review proper eating procedures with workers. Workers removed food, gloves and washed hands.
16: 1770 (A) Slicer was dirty.
Corrective Actions: Clean and sanitize.
21: 0820 (A1) Pork found at 78 F on grill top.
Corrective Actions: Place on time control then discard at 4 hour mark. To correct reheat to 165 F.
22: 0820 (A2) Boba at room temperature. Spring rolls found being prepped but left in styrofoam trays for lunch service.
Corrective Actions: Once a tray is complete, place into refrigerator. Do not keep on counter.
49: 1770 (C) The following found dirty: top of grease trap, lower cookline shelves, mop sink faucet handles, interior of upright freezer.

54: 2740 Outside area still has miscellaneous items.

55: 3170 Mop room floor is in poor repair, entire floor is collapsing.

Many items still require repair/cleaning. On Jan 29 facility is closing so that a new floor can be installed. At that time equipment is going to be moved. Walls behind equipment and floors to be cleaned.
A thirty day permit to be granted to give you the added time to correct the above--floor repair and extensive cleaning.
Two upright freezers, one small make table and a 2 door low boy freezer have been installed.
