6: 12VAC5-421-220 4OUT 220 CORRECTED REPEAT Two (2) twist top bottles of lemonade and one (1) mugs with coffee were observed either on a food prep table with butcherblock and/or on the shelf above the food such as sub bread or large containers of sauces.
Corrective Actions: Correction: Have a designated area for employee drinks (covered with a lid and a straw) away from food prep areas. Ensure employee beverages are handled in a manner to prevent contamination. Coffee mug and screw top drinks were moved ; discussed once opened, any drinks that come in screw top bottles should be put into a container with a lid and a straw such as the to-go cups.

15: 12VAC5-421-470.A.1.a-c 15 OUT 470 CORRECTED Whole Roma tomatoes were being stored in the walk-in cooler a cardboard box that previously held raw chicken.
Corrective Actions: Correction: To prevent cross-contamination, away throw away any boxes that held raw animal products that cannot go be washed and sanitized in the dishwasher or three-compartment sink. Corrected as these tomatoes will be cooked to use in a dish and manager pulled these as they cannot be used for salads or otherwise served raw.

28: 12VAC5-421-3340 28 OUT 3340 CORRECTED A bag of tile mortar was on one side of a shelf right beside/touching bags of croutons. On the other side of the bags of crouton were bottles of member's mark liquid dish soap. This was observed on a shelf in the back storage room near the large mixer.
Corrective Actions: Correction: Please have a designated area away from food/food prep, single-use items like straws and to-go boxes, and clean dishware area to store chemicals, cleaners, etc. to prevent accidental contamination of food. **CORRECTED during inspection by manager moving the bags of croutons**

55: 12VAC5-421-3180.A 55 OUT 3180 REPEAT An accumulation of crumbs, flour, a sticky build up and/or old food particles were observed at the wall floor junction or on shelving in several spots the kitchen such as near three-compartment sink, beside the tall Pepsi cooler, near the prep cooler, the walk-in cooler, and in the dry storage room at the back entrance under / on shelving.
Corrective Actions: Correction Within the next seven (7) days, clean shelving, side of equipment, floors, & walls at a frequency that prevents build up and visible accumulation Also, create a cleaning schedule to prevent the accumulation of grease, dirt, and old food pieces. If you need to open later, close earlier, and/or have staff come in early or staff late something needs to be done to get your food facility cleaner and then you need to create AND follow a daily / weekly cleaning schedule that will keep things clean.

Additional Comments

There is a vomiting and diarrhea clean up kit in the manager's office. VDH food permit is posted on the board on the wall near the bar.

Observed large containers of marinara sauce cooling in pans with ice in the bottom. Discussed ensuring the sauce is stirred as the ice water only touching the bottom 1/3 of the container of sauce. Ensure you cool from 135 F down to 70 within a maximum of two hours. Sauce was still above 150 F when checked during today's inspection.
