48: 12VAC5-421-1640 Observed signicantly soiled sanitizer solution with the solution having large quanity of debris and stained dark brown. made at the time of 9:30 am this morning. Corrective Actions: PIC changed and refreshed sanitizer solution and will inform staff that sanitzer solution shall be replaced whenever it is not clean.
51: 12VAC5-421-2210 Observed mop sink in the rear of establishment having a Y-Valve attached to the sink past the vacuum breaker with both outlets having hoses running with water when not in use. Corrective Actions: When the water hoses are not in use they should be turned off and unplugged as to maintain proper backflow prevention in case of a backup. PIC removed the hose and will educate staff with a training.
