8: 160 Dishwasher observed loading soiled dishes in machine and then unloading clean dishes without washing hands in between, creating a contamination hazard. When I pointed this out to him, he put his hands in a bucket of sanitizer he had at the clean side of the line. I then explained to him that putting his hands in sanitizer was not the same as washing his hands as sanitizer cannot work when hands are soiled. Corrective Actions: Either you need a person to load dirty dish ware and another person to unload clean dish ware, or the person running the dish machine must wash their hands between handling dirty and clean dish ware, each time they make this chnage in duties. This is a reperat violation that was noted with a different dish washer.
28: 3340 There was an uncapped bottle of "Sure Kill CT-511" in the emplayee break area on the same ledge as their water, hot sauce, and A-1. Corrective Actions: Is this a restricted use pesticide that must be applied by someone with a pest applicator license? What is this being used for? Could it be contaminating food contact surfaces. At the very least, it presents a hazard for employees when stored with their drinks and sauces. Told Manager to remove or move to chemical storage area if it is allowable. Would be better for these tasks to only be performed by your pest control company.
28: 3330 Unlabeled yellow chemical in spray bottle under prep sink in dessert area. Unlabeled chemicals could mistakenly be mixed with other chemicals that could be toxic on food contact surfaces or could create toxic gases that could be dangerous to employees or customers. Corrective Actions: COS - Manager found correct label for chemical and put that on bottle. Gave him flyer re: Buffalo Wild Wings issue from 2019. Told him we were working on a simple pictorial flyer about unlabeled chemicals in English and Spanish.
47: 0960 (2-5) Several of the rubber spatulas hanging above the "beef roast" area on the hot line have started to deteriorate and are missing some food contact rubber edges. Corrective Actions: In 90 days (By 12/25/23), remove/replace any damaged spatulas with missing rubber.
53: 2660 Could not find a covered trash receptacle in the ladies bathroom. Corrective Actions: In 90 days (By 12/25/23), have a covered trash receptacle in the ladies restroom.
54: 2730 Middle trash dumpster did not have a drain plug in place. It was also open when not being actively used. Corrective Actions: In 90 days (By 12/25/23), have drain plugs in all dumpsters. Make sure employees know to have dumpsters closed when not actively in use.
55: 3170 On ther grill line, several ceiling tiles had peeling surfaces. This was also noted in dry storage. The floor and coving in front of the walk-in freezer is no longer smooth easily cleanable. To the right of the walk-in freezer, there is a hole in the wall/wall junction, which could be a place where pests could hide. There is missing grout in the floor tile, coving behind the drink syrup boxes across from the employee break area and area is no longer smooth and easily cleanable. Corrective Actions: Repair or replace ceiling tiles. Replace grout and repair areas noted above. This needs to be done in 90 days, by 12/25/23.

Permit posted and good through 12/31/23. VDH QR code sign posted next to permit. Gave Manager cell phone flyer. I did note a lot of cell phones out various places throughout the kitchen. There is potential for contamination if employees don't wash hands after handling cell phones. Gave Manager Rapid Revive training flyer and discussed. Manager says he believes Ecolab has maintained their 2x per week visit and have not received any recent complants. Wet mop head sitting over drain of mop sink and noticed drain flies in area. Please install some mop holders over mop sink so mops can air dry more easily after use. Thermometer inside dessert reach-in has separated. Please replace as I am not sure how accurate the dial thermometer on the outside is. Discussed "trench" depression with green standing water and trash on concrete dumpster pad.
