6: 220 Observed a juice box with a straw inside sitting on a shelf on top of opened canned food items (ex. bamboo shoot strip).
Corrective Actions: Person in charge discarded the juice box. Employee's may have beverages while working, but they must be stored where (if spilled) will not contaminate equipment, food, or single-service items. Re-train employees and monitor/correct as needed.

15: 470 Observed whole, raw sardines over a container of uncovered sliced onions and as well as over individual, packaged to-go sauces in the walk-in cooler. Observed pieces of cut raw beef in a plastic bag as well as a metal container of raw shrimp on a shelf above the following ready-to-eat items: precooked dumplings, cut peppers, and melted butter. Also, cut raw beef over a metal container of soy sauce.
Corrective Actions: Person in charge rearranged shelves according to cooking temperatures.

15: 0580 (A) Observed cook wearing single-use gloves crack a raw egg and proceed to use the same pair of gloves and grabbed a handful of carrots and peas from the prep unit.
Corrective Actions: Cook removed gloves and put on a fresh new pair. Person in charge voluntarily discarded the metal container of carrots and peas from the prep unit. single-use gloves should be used for only one task and should be discarded when damaged, soiled, or when interruptions occur in the operations.

35: 790 Observed a bucket of frozen, raw shrimp in plastic bags submerged in a bucket of water in the walk-in cooler.
Corrective Actions: Person in charge removed the shrimp from the bucket of water and placed the shrimp in a dry container. The dry container wih the shrimp was placed in the walk-in cooler to continue thawing. Proper thawing include under refrigeration, under running water at 70F or less, or as part of the cooking process.

38: 3270 (1) (2) (4) Observed two alive cockroaches on right drainboard of the 3 compartment sink. Observed two dead cockroaches on top of the dishmachine.
Corrective Actions: Owner stated that the pest control agency comes to the facility twice a month. Observed a service inspection report provided from the pest control agency with the inspection date being 12/06/2023.

39: 610 Observed a box of buns, a bucket of raw shrimp, a box of snowcrab, a plastic container of raw beef sitting on the floor of the walk-in cooler.
Corrective Actions: Elevate food items at least six inches above the ground to prevent contamination.
