6: 220 Observed an employee drink without a lid stored on the prep table beside the clean utensil storage rack. (30 days to correct.)
Corrective Actions: Designate an area for lidded employee drinks to be stored away from food and clean equipment.

22: 0820 (A2) Observed time/temperature control for safety foods cold holding above 41F in the reach-in cooler infront of the hot display case. Coleslaw observed at 54F. (Immediate Correction Needed)
Corrective Actions: Cold hold time/temperature control for safety foods at 41F or below. (PIC moved the food to another cold holding unit.)

36: 1320 (A-D) - Some of the refrigeration units are missing internal thermometers. (30 days to correct.) - The thermometer in the hot display case appears to be broken. (30 days to correct.)
Corrective Actions: - Place an accurate thermometer in the warmest part of all refrigeration units. - Replace the thermometer of the hot display case with an accurate thermometer. Place it in the coolest part of the unit.

47: 1570 - Observed ice forming on the floor of the walk-in freezer. (30 days to correct.) - Observed the reach-in cooler across from the hot display case cold holding above 41F. Observed an interior temperature of 56F. (30 days to correct.)
Corrective Actions: - Remove the ice. Repair the unit to prevent the formation of ice. - Repair or replace the cold holding unit to restore an internal temperature of 41F or below. (PIC states the unit will be replaced.)

51: 2200 Observed the spray hose of the 3 basin sink hanging lower than the flood rim of the sink. (Immediate Correction Needed and 30 days to repair more permanently.)
Corrective Actions: Immediately raise the spray hose to create an air gap. (Immediately corrected: Spray hose was raised during inspection.) (In the next 30 days, repair the spray hose to no long allow it the opportunity to fall below the flood rim.)

55: 2810 - Observed tiles missing from the wall behind the fryer. (30 days to correct) - Observed a small crack in the floor between the tiles infront of the door leading to the ice machine closet. (30 days to correct.) - Observed ceiling tiles missing in the men's room. (30 days to correct.)
Corrective Actions: - Replace the wall tiles behind the fryer to be smooth and cleanable. - Repair the floor infront of the ice machine closet to be smooth and cleanable. - Replace the ceiling tiles in the men's room.

55: 3180 - Observed slight cleaning needed at the wall/floor junction below the 3 basin sink. (30 days to correct.) - Observed slight cleaning needed inside the microwave beside the handsink. (30 days to correct.)
Corrective Actions: Clean at a frequency to maintain clean.

56: 3100 Observed employees belongings stored on the storage rack with equipment and on the prep table. (30 days to correct.)
Corrective Actions: Designate an area for employee belongs away from food and clean equipment.
