Corrective Actions: Before washing dishes today, get plain Chlorox bleach for sanitizing.

55: 3170 Observed wall next to toilet is starting to disintegrate, no longer smooth and easily cleanable.
Corrective Actions: In 90 days (by 12/2/22), have this wall repaired. Make sure you figure out what is causing it to disintegrate - leak, window open during rain, etc. to keep it from happening again.

Permit expired 8/31/22. By the end of next week (9/9/22), either bring $40 cash or check to Lynchburg Health Department or call 434-477-5921 and pay by credit card. We are closed Monday for Labor Day. Please call 434-477-5921 to make sure someone will be there to take your money as our Front Office person has left.

Had the Employee Health Policy I gave them last time and signed forms for everyone, including Clover's Mom and Kiki who was there today.
This food establishment was opened with no mop sink because Owner also has a business next door with extra salon sink she uses as a mop sink. Completed Commissary form for this. If you were ever to lose access to that mop sink, you would need to install one, or find one at your current location and complete a new commissary form.
