8: 160 Observed employee handle phone, then go and grab deli paper and gloves to prepare a bagel order. Employee did not wash hands first. Before putting on gloves, hands should be washed.
Corrective Actions: COS - Discussed with person in charge.

10: 3020 The hand soap on the hand sink was so low I could not get any to come out and had to add water to it to wash my own hands. 2 employees were working in the front.
Corrective Actions: COS. All hand sinks should be completely stoacked with paper towels, soap, running warm water if employees are working. Being able to effectively wash hands is a foundation of food safety.

16: 1700 (1-5) (6a) (6b). Sani-bucket with wiping cloth next to hand sink had no sanitizer in it - 00 ppm. Wipiong cloths must be kept in sanitizer solution to keep bacteria from growing on them in between uses.
Corrective Actions: COS - Employee added sanitizer to 400 ppm.

28: 3330 Spray bottle of what appears to be sanitizer next to hand sink was unlabeled. Many chemicals resemble foods and vice versa. It is critical to have working bottles of chemicals labeled to prevent mix-ups that could be extremely dangerous to customers or staff.
Corrective Actions: COS - Asked person in charge to label spray bottle.

37: 900 Establishment now has bagels from Bagel and I. Food labels with allergens were not of packaged bagels and employee could not produce list of ingredients for bagels (although establishment did have for Sweet Indulgence).
Corrective Actions: In 10 days, send me a photo of ingredient list for bagels and make sure they are at establishment and all employees can find them. If you do not make the foods in house, and they are not labeled on the packaging, you must have a way to tell customers the ingredients.

45: 2000 (A) (C) Box of cup jackets, box of coffee stirrers and bags of coffee beans on floor in dry storage. All single service items (as well as food) must be stored 6" off floor.
Corrective Actions: As soon as possible, but no later than 90 days, have dry storage organized so no single service, food is on the floor.

Additional Comments

There was a big cooler and mitten cookies, cake pops in back. Employee states these are for ice skate and say they have Sweet Indulgence ingredient list at the ice skate venue.
Has Maintex Sani-Cide and test strips good through 5/26.
Gave person in charge new allergen hand out.
