*15: Observed an unlabeled spray bottle of green liquid hanging on a bin full of clean laundry. Observed spray bottles sitting on the top of the houskeeper carts directly next to clean linenes and single use items- mUst be stored bellow or to the side.
*20: Room 308 and 408 do not have exmergency evacuation maps present. Will return sometime within 10 days to ensure the correction of the observed critical item violation. Rooms should be blocked until emergency evacuation signs are hung and the correction is verified by tdh.
33: Room 121, top sheet has some yellow spots
Room 218: top sheet has some holes.
35: Room 308: microwave dirty and appears to also have some rust in the inside.
Room 412: stains on the grey curtains
Rooms checked: 117, 121, 218, 215, 312, 308, 412, 408, laundry room.
Will return sometime within 10 days to ensure the correction of the observed critical item violations 15 and 20.
