5: Ice meching on first floor is leaking from the front and back of machine.

*15: Housekeeping cart in laundry room: unlabed bottle of pink liquid, and an unlabeled/unsealed bottle of green liquid on top of cart. Chemicals are stored with single use items and above clean towels and linens.

*18: Smoke detector across from laundry room is hanging detatched from the wall and does not work when tested.

*18: Fire extinguisher #16 (between rooms 131 and 133) does not have a breaker bar. This is the 3rd consecutive time this priority item violation has been noted, a permit revocation request will be submitted. Fire extingisher #10 on second floor has a burnt tag, unable to verify date.

*20: Room 214, 207: door not self closing. Also the evacuation sign is not properly labeled. (See photo)This is ther second consecutive inspection this has been noted, a warning letter will be requested. 206: evac plan not labeled.

24: Rolls of carpet stored in walk ways on forst floor. Multiple desks, dressers, and lighting stored in hallways outside of rooms on second floor; beds and bed frames stored near stairwell On second floor; construction debris in halway of second floor. Carpeting on both floors’ hallways in poor repair.

25: Room 112: ceiling tiles in bathroom in poor repair, and black residue along ceilding tiles and caulking. Room 102: bathtub has large yellow stain

29: 207: entry light, Desk light, tall lamp, and wall lamp near desk are all out 204: wall lamp doesnt work

32: Room 111: only one wash cloth.

35: Room 112: only one seat , does not have fire resistant curtains. Room 111: tall lamp is rusty Room 108: only one seat, large mirror and desk appear dirty. Room 108: only has one seat Room 214: only one chair, sliding door excessively dirty, curtains heavily stained on white side. Room 207: curtain heavily stained on white side and has large tear. 206: sliding door dirty and white side of curtain stained and dirty. 204: large tear in curtain.

37: Room 111: ceiling around edges peeling, walls and ceiling appears to have some damage around the window. Room 108: walls around window damaged! Ceiling peeling. Room 214: walls around ac unit in poor repair Room 206: wall paper coming off the walls

*42: Room 112: Small body hairs all over fitted sheet and loose sheet. Body hairs and small stains on pillow cases.

Owner stated that renovations were being done - a full plan, including changes, specs, and material lists, MUST be submitted to our office for approval as stated in the TN regularing regarding renovations and changes.


A follow up will be done within 10 days to ensure all critical items marked in red are corrected, item 18 has been out of compliance 3 consecutive times and item 20 for 2 consecutive times - the apropriate revocation requests will be submitted.
