11: Observes a bin of onions in prep cooler #1 decaying/in damaged condition CA: embargoed 2lbs of onions

14: Sanitizer bucket in at autochlor dishmachine was very low and barely pulling Sanitizer with chlorine at 0ppm. Observed leak coming from bottom of dishmachine CA: swapped out with new sani bucket with ppm still reading 0ppm. 3 comp sink setup

36: Observed a small fly land on unprotected pizza sauce in ice well

37: Fan guards in reach in cooler #1 need to be cleaned

45: Racks in walk in cooler have food debris building up and need to be cleaned

Additional Comments

Observed no vectors that could lead to a food borne illness outbreak. Dishmachine has leak but owner stated that leak was noticed yesterday and autochlor us coming out today(11/28/2023) to investigate leak and repair dishmachine. No employees observed being sick with no employees reporting to be sick in last two. Weeks. Roof leak was repaired two months ago with new roof being installed this month. Prep room from complaint is actually a neighbors storage unit, tried peaking into storage room and but couldn't didn't tell or see any food prep/storage. Will need to gain access to get more information
