Observations & Corrective Actions

12: Observed raw mussels in walkin cooler without tags, and observed a sea food mix of raw mussels, raw shrimp, and crab - no tags kept with the mixture. Pic stated they get some mussels in a large case filled with 12 smaller cases. Had the tags for the largs box, information on smaller boxes match the large one, some of the 12 boxes remained so “last date used” was not present. COS by embargoing the small, tagless container (1lbs) and the sea food mixtures (20lbs). Discussed tags must follow the raw mussels as well. Suggested keeping a cut out of the small boxes with any mussels that are removed from the boxes, and whenever possible leaving the mussels in the original containers.

12: Observed establishment keeping shelstock tags without writing the final use date on the tags, no way to verify the 90day requirement for keeping shellstock tags. COS discussed requirements with pic, will include State Regulations regarding shellstock in email. A warning letter will be requested due to violation being noted on two consecutive routine inspections.

12: Observed an open box of frozen, raw oysters 8n the walk-in freezer without a tag. pic stated the tag was with in-use portion in the walk-in cooler. No other information related to shelstock tags was on the open box. COS by embargoing the un-tagged portion of the oysters, approximately 5lbs, and discussing proper upkeep of tags when portioned out of the original container.

18: Observed cooked shrimp in the walk-in cooler above 41°f. Pic stated the small portion was cooked yesterday, cooled with an ice bath, had been inside the cooler since 8pm yesterday. all other items prepared at similar times were within proper temperature.corrected by embargoing 1lbs and discussing. A warning letter will be requested due to violation being noted on two consecutive routine inspections.

33: Observed a large container of frozen shrimp left out on the counter to thaw

37: Cases of food left stored on floor in the kitchen post delivery/during prep.

53: Floor tiles are in poor repair in the kicthen, ceiling tiles in poor repair/ have debris build up throughout the kitchen.

55: Has expired permit posted

Additional Comments

A warning letter will be requested due to violation 12 and 18 being noted on two consecutive routine inspections. Will provide applicable fact sheets to operator regarding Priority Item Violations via email. Discussed implementing a Food Safety Plan (FSP) in order to obtain active managerial control over repeated priority violations. Advised operator I was available to assist with developing a FSP.
