11: Several lettuce heads were browning in walk in cooler. Mold on container of strawberries

14: Ice machine has pink slime present. Please wash, rinse and sanitize ice machine.

20: Raw Shrimp,raw beef and broccoli are not reaching temperatures of 41 F or below. Walk-in cooler and prep cooler are not reaching a temperature of 41 F or below.

37: Container of chips were uncovered. Uncovered food in prep cooler. Container of Broccoli was on floor in kitchen when I arrived.

42: Ice scoop and food scoops are not properly stored. Ice scoop was stored on top of ice machine. Rice scoop was stored on top of rice container. Please keep scoop covered .(zip lock bag or container )

45: Ice build up in deep freezer. Microwave is dirty. Please wash, rinse and sanitizer microwave

47: Metal Shelves have grease build up.

53: Build up on walls , ceilings and floors in kitchen. Damage floor tile in dining area by bathroom.

While i was onsite a customer complained that there was a fingernail in her food. I did witness manger getting the fingernail out of customer food.
