Observations & Corrective Actions
6: Employees never washed their hands after entering kitchen or when changing tasks. Discussed hand washing with pic.
11: Dented can on top and bottom seem of can and celery was brown in wic. Pic discarded both.
19: Brown rice in steam pot was hot holding below 135°F. Pic threw rice away.
20: Bagged noodles were stacked above the rim of the pan located on prep cooler near the grill and was reading above 41°F. Bags of noodles were discarded by pic until temp was reached of 41°F of bagged noodles. Cook stated that he restocked the prep cooler about 20 minutes ago. Do to this priority item being noted on 3 consecutive inspection reports in a row, permit may be up for revocation.
36: Several flies observed near drink station at the bar and back of kitchen where the dish machine is located. Did not observe any live ants or roaches. There were several dead roaches observed behind a table at the front of the house.
37: Bag of sugar stored on the floor opened near old drink station.
41: Ice scoop stored on top of dirty ice machine.
45: Build up of dirt and debri on can opener.
45: Steam pots near grill have electrical wires exposed and handle missing. Lid for rice pot in the back is missing a handle.
51: Men’s restroom appears dirty and the light was out above the toilet. Paper towel dispenser was damged, but paper towels were available for people to dry their hands. Women’s restroom did not have any toilet tissue in one of the stalls.
53: Several seats in the dining area are damaged.
53: Several damaged walls in the back of the kitchen. Water leak from the ceiling near the dish machine. Missing ceiling tiles near ice machine / prep area. Some wall tiles are missing as well.
53: Wall below where you leave your food to be cooked is dirty.
54: No cover over light above 3 comp sink and missing some light bulbs.
Additional Comments
Received two complaints on this establishment. First complaint - There was roaches in the dining room and crawling on the tables. Second complaint - tables in dining room were sticky and dirty and there were ants crawling on them, windows were dirty, area where you make your bowl of food was dirty, cups they served drinks in were dirty. The bathrooms were dirty, trash cans were overflowing with trash, soap dispenser and towel dispenser was broken. Provided applicable fact sheets to operator regarding observed priority item violations. Discussed implementing a Food Safety Plan (FEP) in order to obtain active managerial control over repeated priority item violations. Advised operator I was available to assist with developing the FSP. Permit may be up for revocation due to priority item violation 20 being noted on 3 consecutive inspection reports in a row. A warning of permit revocation will be issued due to priority item violations 6 and 11 being noted on 2 consecutive inspection reports in a row. A follow up inspection will be performed within 10 days to verify compliance of all priority item violations. Observations observed from complaint are noted in the violation section of the inspection report.
