1: 1200-23-01-.02(1)(c) No managerial control observed at time of inspection, based on the number of avoidable critical violations observed. CA: strongly suggest basic food handlers class to PIC; follow up needed.

2: 1200-23-01-.02(2)(a)2. Present employees could not recall nor provide employee health policy. CA: left copy.

4: 1200-23-01-.02(4)(a)1. Observed cook eat chicken from the counter top cooler with gloved hands then proceed to cook food on grill top with same gloved hands. CA: discussed with PIC and employee about where to eat and drink and when to wash hands.

4: 1200-23-01-.02(4)(a)1. Observed employee drink from open can, while cooking food at fryer. CA: discussed with employee and PIC about proper places for employees to eat or drink.

6: 1200-23-01-.02(3)(a) Observed cook enter kitchen from outside with gloved hands and prepare cooked food without first removing gloves, washing hands, and placing new gloves on. CA: discussed proper hand washing practices with employee and PIC.

7: 1200-23-01-.03(3)(a)1.(ii) Observed cook throw fry that had fell on to table from seasoning bowl back into bowl of cooked fries with bare hand. CA: discussed bare hand contact policy with employee and PIC. Embargoed bowl of fries.

11: 1200-23-01-.03(1) Observed raw beef packaged thawing in 3 comp sink, in bowl, observed container of soapy water stored beside. Observed dishwasher dunk sponge into adjacent bucket splashing soapy water into bowl of raw beef. CA: embargoed raw beef; discussed proper sink and thawing set up with employee and PIC.

14: 1200-23-01-.04(6)(a)1. Observed employee only washing food dishes with soapy water and rinsing with clean water prior to drying. CA: trained employee to wash, rinse, and sanitize all food contact surfaces.

21: 1200-23-01-.03(5)(a)7. Observed larger container of cooked chicken stored inside reach in cooler 3 without any date markings. Cooks unable to recall when product was made. CA: discussed importance of dating cooked items kept longer then 24 hours, embargoed chicken.

Additional Comments

Complaint: Customer stated and provided video of dog, which did not appear to be a service animal, roaming around bar.
Answer: Observed leashed dog sitting at bar upon entering establishment. Spoke with PIC about animals inside of food establishments as well as ADA FAQ.
