36: Several live roaches observed in lobby and near single service items. Several dead roaches were observed in employee bathroom and corners of the kitchen. Several live and dead gnats observed in employee bathroom on a green service towel (see photo). PIC stated and provided proof of pest control contract currently in place.

37: Personal fanny pack was observed on food prep table in back of kitchen near WIC.

42: Several dishes located on the clean dish rack in the warewashing area were dirty and still wet from yesterday. According to PIC the dishes were washed yesterday.

53: Air vents near pizza oven has excessive buildup around vents. Ceiling tile missing in employee bathroom. Hole in drywall in the employee bathroom. Floor tiles are damaged throughout the kitchen. Wall panel is missing near hotboxes in the vicinity of the front counter. Ceiling tiles stained throughout establishment. See photos.
