6: 1200-23-01-.02(3)(a) Main cook was observed coming out of the bathroom and immediately went back to cooking food. He claimed he washed his hands in the bathroom. Proper hand washing procedure was explained. Employee must wash hands in the bathroom and also wash hands where everyone can see them / before returning to food prep area.

8: 1200-23-01-.06(3)(a) There was no soap at handwashing sink in the back area

21: 1200-23-01-.03(5)(a)7. Chicken that was cooked on saturday in Reach In Cooler does not have a date mark. Corrective action, 24 hours date marking was discussed.
