*9: Presence of multiple dead and live small roaches observed on floor of room 132. Instructed to close room and have it serviced by pest control. Follow up required.

11: Observed wadded damp washcloth in sink of laundry area harboring abundant small fruit fly like insects.

*15: Three chemical spray bottles with bubbly yellow and blue liquids not labeled in laundry room. Corrected on site.

25: Observed brown stained floor tiles near toilet of room 131.

25: Missing and damaged floor tiles in room 132.

32: Observed wash cloth excessively stained in room 132.

32: Missing clothing hangers in room 132.

33: Several cigarette burns observed on bed sheets of room 132.

33: Mattresses not protected with fitted sheet or bed bug cover.

35: Observed curtains in poor repair room 132.

37: Observed several large stains, blemishes, and peeled paint on wall of room 132.

Additional Comments

Answering complaint received 9-4-24 during inspection where person claims there were bugs everywhere in room 132. Person in charge states they are serviced by total pest control. No presence of bed bugs observed, I did observe multiple dead and live cockroaches. Closed room to be serviced with follow up required.
Fire extinguishers serviced April 2024. Allows smoking.
