Observations & Corrective Actions

1: No certified manager present, unable to discuss all aspects of basic food safety, active managerial control needed due to high amount of repeated priority violations. A warning letter will be requested due to violation # being noted on two consecutive routine inspections.

6: Observed pic wash dishes with gloves on and return without changing gloves/washin hands. Also observed employee wash hands in the 3 compartment sink instead of the handsink beside it. Corrected by having pic wash hands before touching food, and discussed proper handwashing. After, watched employee take a drink in the driving area from a plastic water bottle and go to return to cooking without washing hands. Made pic aware and employee washed hands.

8: Observed a bag of radishes stored in the handsink, blocking the use of the sink. Corrected by pic moving the food, and discussing. A warning letter will be requested due to violation being noted on two consecutive routine inspections.

14: Observed 3 comp not set up, pic wash rice pot without setting up the sink, rinsed, and then wiped pot with hand sanitizer. Discussed proper set up. Pic unable to properly wash/rinse/sanitize due to not having sink plugs (caps). Discussed pic should purchase before the next time they operate. Will return sometime within 10 days to ensure the correction of the observed priority item violation This item has been noted on 3 consecutive routine inspections and revocation paperwork will be submitted to supervisor.

19: Observed the pot of rice at 122°f, pic stated it was made in their home at 7am and then moved onto the truck before coming to the event. Corrected by embargoing appropriately 10lbs of rice, discussed food can only be prepared on the mobile unit or another permitted commissary kitchen and cannot be prepared in home. Will return sometime within 10 days to ensure the correction of the observed priority item violation A warning letter will be requested due to violation being noted on two consecutive routine inspections.

20: Observed fried rice and shelled eggs in the front reach in cooler that had an ambient temp of 52°f. Pic stated they knew it did not work and don't have food in it. When asked how long the rice and eggs have been in there, pic stated “old” as the answer. RIce at 62°f and shelled eggs at an ambient temperature of 52°f corrected by embargoing 3 pounds total, 2 of rice and one of eggs. Discussed proper holding temperatures. This item has been marked out of compliance 3 consecutive routine inspections, revocation paperwork will be submitted to supervisor.

23: Observed raw bluefin tuna on the unit, pic stated it is served raw in the tuna poke - “tuna poke” on menu states it is made with cooked salmon. Unable to correct on site. Discussed that the menu item must state the raw tuna, and that the consumer advisory and disclosure must be on the menu. Will return sometime within 10 days to ensure the correction of the observed priority item violation

33: Left frozen bag of general tso-like chicken thawing next to the fryer, food below 41°f. Discussed and placed back under temperature.

37: Containers of food and water coolers stored directly on the floor - should be elevated.

44: Pic reusing/washing gloves instead of changing.

Additional Comments

Will return sometime within 10 days to ensure the correction of the observed priority item violations marked between 1-27.

A warning letter will be requested due to violation # 1, 8, and 19 being noted on two consecutive routine inspections. Will provide applicable fact sheets in Thai to operator regarding Priority Item Violations via email. Discussed implementing a Food Safety Plan (FSP) in order to obtain active managerial control over repeated priority violations. Advised operator I was available to assist with developing a FSP.

Will submit revocation paper work to supervisor due to items 14 and 20 being marked out on 3 consecutive routine inspections. Discussed importance of answer emails from TDH members and providing an updated schedule.

Place 7 individually vacuum sealed bags of raw bluefin tuna on hold, only one of seven had a label. label was damaged and could not verify thawing instructions. Pic stated all from same bulk and provided receipt of 10ct. Discussed all 7 items to remain on hold until they can provide proof of thawing instructions to our department. Tuna is not to be used or discarded until hold order is lifted by a TDH member.
