14: Observed the 3 comp set to soak and the dish machine read 0ppm cl after running two times in a row? Pic attempted priming Dish machine on the 3rd run and we both had a 0ppm cl reading in our test strips. cos pic had employee start filling the sanitizer side of the 3 comp and called the service company. Employee checked the lines and noted an area where the tube to the sanitizer appeared blocked, tube was adjusted, dish machine ran and got a 100ppm reading.

37: Observed at the grill, raw chicken stored in the top drawer above raw shrimp in the bottom drawer. Employee’s phone stored on shelf with plates.

41: Handle of ice scoop touching ice. Using a bowl as a scoop in container of diced tomatoes and the rim touching tomatoes.

Additional Comments

Item 14 has been noted on 3 consecutive inspections, the revocation request will be submitted as standard operating procedure.

Will Provide applicable fact sheets to operator regarding Priority Item Violations via email. Discussed implementing a Food Safety Plan (FSP) in order to obtain active managerial control over repeated priority violations. Advised operator I was available to assist with developing a FSP.
