8: Handsink next to Walk in cooler had hot water turned off due to leak. Corrective action - hot water must be supplied to handsink. Manager turned on. Will recheck sink in 10 days.

11: Several severely dented cans on good can shelf. Corrective action - retrained and pic removed

11: Piece of large plastic foreign debris found inside bucket of cooked pork. Corrective action PIC embargoed 30 lbs.

13: Raw chicken stored over Ready to eat pasta and limes in Reach in cooler . Corrective action - trained and pic moved chicken to bottom shelf.

13: Raw chicken stored over uncovered mac & cheese in Walk in cooler . Corrective action - pic embargoed 10 lbs of mac and cheese.

21: Date marks not present on cooked chicken, pasta, or other foods in Walk in freezer

36: Screen on back door broken allowing insects inside

45: Concrete mixer previously used for egg roll making stored in kitchen . Concrete mixer cannot be used for food prep and should be removed from the establishment .

47: Dead cockroaches observed in back ice machine. Spoke with manager about cleaning and sanitizing machine before use.

Complaint answered: (received on 4/5/23, answered on 4/5/23)
Complainant stated that they ate at the establishment on 4/4/23 and water was dripping from the ceiling onto the food and area that they were eating in.Complainant stated that they felt unwell after eating the food that was contaminated with unknown water from ceiling.
Observations: Vents in dining room area are not leaking water. Manager stated that they had the air conditioner on before the customer came in but shut it off a little bit before closing. The air vent over the customers table was leaking onto the customers food and table. The staff asked him to get a different plate and offered him a different table but they stated that he wanted to stay at the table and continue eating .
