21: Observed a bin of 3 italian cheese mix date marked to be pulled on 09/08/24(todays date is 09/09/24) CA: embargoed 2lbs of cheese product

22: Shredded lettuce in Prep Cooler #1 temped at 53F, lettuce was labeled to be rotated at 2:30pm(time when temping was 3:15pm) Person in Charge stated that Shredded lettuce are rotated every 4 hours per Time as a Public Health Control policy CA: embargoed 2 lbs of food product and had Person in Charge rotate fresh lettuce into Prep Cooler #1

34: No working thermometer in open top raw beef cooler

36: Observed a large presence of small and large flies throughout facility

36: Back door open upon arrival for complaint

37: Bags of sugar in dry storage are not properly sealed and stored causing flies to be attracted in dry storage

46: Leak coming from 3 compartment sink faucet causing water to drip into rinse compartment

50: Observed a floor drain in back salad prep area with missing cover and standing water

53: Observed large amounts of food debris and grease building up on ground and walls underneath equipment in kitchen

Additional Comments

Complaint: upon arrival for inspection I observed the back door propped open, when walking into dining area observed multiple small flies; during walk through before starting inspection a multitude of small and large flies could be seen all over kitchen and dining space.

I noticed two prominent areas that they're coming from:
1) uncovered floor drain in back area near salad prep station and 3 compartment sink was observed to have standing water with flies all around.

2) in sugar storage area near ice cream machine there was a large pile of sugar on ground with sugar bags open and flies all over sugar.

Person in Charge stated that ecolab pest control has been lacking and they haven't been conducting monthly pest control for establishment. They were noted to last be at establishment at the beginning of august. Person in Charge stated they've reached out about pests recently as last week with no reply from ecolab.

Follow-up will be conducted once kitchen has been cleaned of food debris build up throughout kitchen and pest control has come out to mitigate and deal with pests
