1: Person in charge does not have a current Food Manager Certificate. Several foodborne illness risk factors out of compliance. Additional training for manager and staff is required.

14: Sanitizer bucket was observed empty upon arrival and dispensing at 0ppm in machine. This was corrected during inspection and is sanitizing at 50ppm at end of inspection.

26: 1200-23-01-.07(2)(a) Observed driveway concrete cleaner stored on shelf next to liquid butter. Store toxics in separate area from food products.

47: Debris observed on handle of walk in refrigerator.

47: Accumulated food debris on outside of bulk food bins. Clean and sanitize on a more routine basis.

49: Provide air gap from wastewater pipe coming from 3 compartment sink to floor sink. Also, discard rag connected to faucet at three compartment sink.
