22: TPHC logs are incomplete . The last TPHC policy observed in the Food log binder is dated 12/2/22. It is now 1/18/23. Manager stated that the head manager is out and when she was given paperwork from administrators, it was supposed to be a new Tphc form but the form does not support the policy. Corrective action - trained manager on proper tphc form guidelines.

49: Fresh water leaking from water filtration unit in kitchen area.

50: Dumpsters uncovered and trash and old mattresses and other furniture surround the dumpsters.

55: 2020 permit posted.
