11: Raw steak in cooler was discolored. Pic unsure how long the raw steak had been in the cooler drawer. Pic threw raw steak away.

18: Sliced tomatoes prepped this morning at 10:00 was temping 48°F. Embargoed tomatoes. Dicussed proper cooling procedures.

20: Salsa on ice was temping 55°F. Pic stated salsa was brought out this morning around 10:00 am from wic. Embargoed salsa.

36: Opened cooler drawers below grill and several different size roaches came scattering out. Pic threw away food in cooler drawers. Contact a licensed exterminator to treat establishmentq for roaches.

45: Couple knives had rust on them. Large cutting knife extremely damaged. 2 spatulas extremely damaged.

54: Lights not working in the dish area.

55: Permit posted is expired.

Additional Comments

Provided applicable fact sheets to operator regarding observed priority item violations. Discussed implementing a Food Safety Plan (FEP) in order to obtain active managerial control over repeated priority item violations. Advised operator I was available to assist with developing the FSP.
A follow up inspection will be performed within 10 days to verify compliance of all priority item violations. A warning letter will be requested due to priority item violations 11 and 20 being noted on 2 consecutive inspection reports in a row.
