*27: Tattoo artist (Nicholas) went to truck to get license, came back in and put on gloves, then started tattooing a patron without washing his hands. he did not have any soap at his hand sink. Jacob did not have any soap at his hand sink, he said he had hand sanitizer.

35: Several ink bottles expired in Milt's and Nicholas' tattoo booth.

Additional Comments

Failure to correct any critical item violation within 7 days and non critical item violation within 14 days could result in suspension of Tattoo Permit. A civil penalty will be assessed of $125.00. April 10-14, 17-20, May 4-9, May 19-23, June 19-27j.
