6: 1200-23-01-.02(3)(a) Did not observe any hand washing by cook throughout inspection and between jobs. Second notice letter will be ordered.

13: 1200-23-01-.03(3)(b)1. Raw beef steaks stored above raw fish in lowboy cooler. Cos by moving items to correct storage areas.

19: 1200-23-01-.03(5)(a)6. Chicken held below flat top temped at 102 degrees. Pic stated chicken breasts are cooked every morning and held through lunch. COS by reheating all chicken to 165 or above and placing on steam table.

20: 1200-23-01-.03(5)(a)6. Food in prep cooler temped between 45 and 59. The raw beef at 59 was cooling from being cut this morning, but everything else had been pulled from the WIC this morning. COS by moving items to working cooler and using ice baths on items that remained in cooler.

37: Employee cellphone stored in box next to bread being used for sandwiches. Foods uncovered in freezer.

39: Wiping buckets being left on the floor.

53: Lights in hood need repairs and covers replaced. Ceilings in kitchen have falling tiles and build up of debris on vents.
