7: 1200-23-01-.03(3)(a)1.(ii) Employee was observed pressing his phone and immediately went back, put on gloves and strat preparing food without washing his hands. Corrective action, proper hand washing procedure was explained to employee.

8: 1200-23-01-.06(3)(a) There was no paper towel at handwashing sink beside dishwasher. Corrective action, person in charge supplied paper towel.

13: 1200-23-01-.03(3)(b)1. Raw eggs and raw beef stored above milk and sour cream in rented mobile cooler. Corrective action, proper vertical storage of raw and ready to eat food was discussed.

20: 1200-23-01-.03(5)(a)6. Raw Beef, raw pork, raw chicken, and raw fish in grill cooler at 64f. Person in charge does not know how long this cooler has been turned off. Cooler was reading 64f when turned on. Corrective action, embargoed 40LBS.

21: 1200-23-01-.03(5)(a)7. Person in charge is not sure when rice in mobile cooler was cooked. Corrective action, embargoed 10LBS.

26: 1200-23-01-.07(1) Bleach water inside white chemical spray bottle in front of dishwasher is not labeled. Corrective action, person in charge labeled spray bottle.

34: There was no thermometer in mobile walk in cooler

36: Live roach seen crawling on wall in dry food storage area

37: Shrimp, beef, chicken, and rice stored directly on the floor in rented cooler.

Additional Comments

During this inspection, it was observed that walk in cooler and walk in freezer were out. Establishment was using a rented mobile walk in cooler.
