11: Observed severely dented can of mushrooms on shelf of prep table beside hand sink. CA: embargoed 6 lbs.

19: Grilled chicken in steam well temped at 106F at time of inspection. Chicken had been on steam table for less than 4 hours. CA: reheated chicken on grill and turned up temperature of steam well.

19: Cooked rice temped at 103F at time of inspection. Employee stated the rice was cooked today, but the steam table broke. CA: reheated rice on grill and stored it inside metal container on the grill.

20: Chopped iceberg lettuce inside reach in cooler 2 temped at 45F at time of inspection. Date mark on lettuce is 4/28/24. Thermometer inside cooler is reading 46F. CA: embargoed 5 lbs of lettuce.

21: Several containers of cooked chicken and cooked gyro meat stored in reach in cooler 3 for more than 24 hours do not have date marks indicating when they were cooked. CA: trained employee on date marking.

34: Missing thermometers in multiple coolers and freezers.

37: Observed employee food stored with establishment food inside lowboy cooler.
