14: Low temp dish machine dispensing 0ppm Chlorine sanitizer even after multiple washes and sanitizer container full. Person in charge says possible air gaps present as it was having issues prior. Follow up required within 10 days. Operation is to use 3 compartment sink to sanitize.

14: Multiple knives stored clean on magnetic strip observed very dirty with food buildup. Corrective Action: removed to dish area to rewash and sanitize.

22: Observed container cooked rice noodles left at room temp not time stamped. Corrective Action: corrected on site.

45: White cutting boards on prep cooler observed very worn with deep knife marks.

47: Interior shelving of multiple coolers including walk in cooler observed dirty.

53: Multiple ceiling tiles missing above ice well and tea.

53: Observed several areas of wall containing black buildup and food stains throughout kitchen.

53: Observed on going issue of excess water spilling over accumulating onto floor from cookline.

57: Require no smoking signs on every entrance and exit door.
