8: Observed hand sink at the bar with no paper towels. Cos pic brought over napkins, discussed implementing a permanent fix of paper towels.
A warning letter will be requested due to violation being noted on two consecutive routine inspections.
13: Observed uncovered pans of raw beef stored directly over and open Bag of french fries and an open pan of tamales. No obvious signs of contamination visible. COS discussed with pic and pic had employees rotate all raw to the bottom and ready-to-eat on the top. A warning letter will be requested due to violation being noted on two consecutive routine inspections.
21: Observed a 5 gallon bucket of queso dip without a date marking label - pic stated it was made Saturday and since they were closed Sunday, no label was placed on it. Also observed a small pan of beef fajita mix without a dare mark and pic stated it was also made Saturday. COS embargoed approximately 15lbs of queso and 1 lbs of beef fajita mix and discussed proper date marking. Suggested placing dates on all items at closing on Saturday to avoid date marking issues. A warning letter will be requested due to violation being noted on two consecutive routine inspections.
23: New menus - consumer advisory posted but disclosure (* symbo” is not present on the ribeye and t-bone menu items that can be ordered under cooked. Will return sometime within 10 days to ensure the correction of the observed priority item violation

Will provide applicable fact sheets to operator regarding Priority Item Violations via email. Discussed implementing a Food Safety Plan (FSP) in order to obtain active managerial control over repeated priority violations. Advised operator I was available to assist with developing a FSP.
