*15: Observed an unlabeled Chemical spray bottle of purple liquid hanging on the first floor cart. House keeper identified as fabulouso and labeled. A warning letter will requested due to violation being noted on two consecutive routine inspections.

*18: Room 222: smoke detector not working - no light and no sound when test button pressed A warning letter will requested due to violation being noted on two consecutive routine inspections. A warning letter will requested due to violation being noted on two consecutive routine inspections.

*20: Room 107: door Is not self- latching A warning letter will requested due to violation being noted on two consecutive routine inspections.

*20: Room 142 and 209: only one emergency phone in the room and it does not have a dial tone. A warning letter will requested due to violation being noted on two consecutive routine inspections.

25: Room 209: clump of hair left on the shower walls

25: Room 133: large area of black substance on ceiling above shower

33: Room 215: light staining on the beds’ fitted sheets

33: Room 142: cigarette burns in the top blanket.

*42: Room 222: bed closest to the bathroom has dirty linens - covered in body hair at top and bottom of the sheets, debris in bed between sheets, and what appears to be fresh stains
