21: 113- toilet tissue holder coming loose out of wall in bathroom.
25: 148- bathroom light cover is missing.
26: 133- caulking around barhtub is in poor repair.
29: Room 148- lighting above beds do not work. Room 133- main ceiling lighting by window has hole in it.
33: 113- bed mattress pads and bed stained.
34: 148- pillows stained 133- pillows stained
36: 123- dark stain on carpet infront of bed closest to entry door.
37: 133- hole in wall next to entry door. 117- ceiling is peeling in various areas.
A complaint was called in regarding room 122. Complainant observed bed bugs, lamp with loose wiring, holes in the walls, dirty walls, flies present.
I inspected room 122. I did not observe any bed bugs for both beds. Walls did have stains on them. All lighting was working properly. No flies were present.
