8: Hand sink beside three comp sink did not have paper towels. Back hand sink was blocked by a trash can and cart, and there was not any hand soap. Back hand sink was extremely dirty. Everyone was using front hand sink to wash hands.

26: Spray bottle with clear liquid inside located near 3 comp sink was not labeled.

37: Several employee drinks stored above pizza make line along with cell phone.

41: Handle of scoop was touching precooked sausage on pizza make line.
