6: Observed dish washer load dirty dishes in machine than handle clean without Washing hands first. Cos by having pic duscuss hand washing in between.

13: Bread loaves that is cut and served are stored in cardboard box. Cos by discussing food contact surface should be sanitary. Embargoed 6 loaves.

14: Dish machine reading 0 ppm of cl. Cos by poc changing bucket. Now reading 50 ppm.

23: Establishment has consumer advisory but has no denotation besides foods that are cooked to order.

26: Sani bucket stored directly beside clean plates.

37: Employee drink stored on top of dish machine.

47: Damaged door on glass warmer.

Additional Comments

Pic stated they are using the wic at the flipping axe which is a permitted kitchen. Pic stated they are only storing cooked foods to be used in catering events. No food prep can be done under a different permit holder.

Discussed implementing a Risk Control Plan (RCP) in order to obtain active managerial control over repeated Priority items. Advised operator I was available to assist with developing the RCP

I will return within 10 days to verify all priority violations have been corrected.
