6: Observed no time records for the pizza and calzones sitting out at room temperature on the serving line.
Corrective Actions: Must provide proper time control procedures.

9: Observed raw shell eggs stored above onions in the walk in cooler. Observed raw shell eggs stored above whipping cream in the reach in cooler by the grill.
Corrective Actions: Must properly store raw foods.

22: Unable to provide food handler documentation for the employees on site.
Corrective Actions: Must obtain food handler documentation for all food handling employees.

28: Observed no date markings on shredded mozzarella, pasta sauce, and cut veggies in the walk in cooler.
Corrective Actions: Must date mark all food items that are kept for longer than 24 hours.

29: Observed no thermometers on the inside of the reach in coolers that are located by the grill area and underneath the salad station.
Corrective Actions: Must provide thermometers inside the reach in units.

31: Observed a soiled wiping cloth on the hand sink that is located to the left of the coke cooler.
Corrective Actions: Must use hand sinks for hand washing only.

32: Observed a broken gasket on the standing reach in freezer that is located to the left of the stoves. Observed what appeared to be mold on the gasket crevices on the walk in cooler door.
Corrective Actions: Must repair/replace.

36: Observed multiple wiping cloths stored on the preptables throughout the facility.
Corrective Actions: Must store wiling cloths in the sanitizer solution when not in use.

37: Observed what appeared to be dust on the vent covers in the walk in cooler.
Corrective Actions: Must clean.

37: Observed multiple boxes of food stored dirctly on the floor inside the walk in cooler.
Corrective Actions: Must have food stored at least 6 inches above the floor at all times.

43: Observed no light shield on the ceiling light located in the dishwasher area.
Corrective Actions: Must repair/replace light shield.

45: Observed multiple floor tiles missing throughout the facilitiy.
Corrective Actions: Must repair/replace floor tiles.

47: Observed no hand washing signage or labels at the designated hand sink that is located to the left of the coke cooler. Observed no employee must wash hand signage in the restrooms.
Corrective Actions: Must label hand sink. Must provide proper signage.
