4: 113977, 114256 (a) Observed water bottle with unapproved screw-top lid stored on prep counter. Provide a lid/straw and store below/away from any food or food prep areas. Corrected on site.

14a: 113984(d), 114113, 114115, 114117, 114121(a), 114125(b), 114141, 114180(a-c) Observed reusing stainless steel for mixing cracked raw shell eggs without wash, rinse and sanitize (stored on prep counter). Discontinue reusing stainless steel for mixing raw eggs. Recommend premix batches of cracked raw shell eggs in food grade container and stored in refrigerator maintaining 41F or below for later used or use new container for every new cracked raw shell eggs. Correct immediately.

29: 114254, 114254.1, 114254.2 Observed 1 unlabeled spray bottle hanged next to hand sink. Label all spray bottles. Correct immediately.

30a: 114047, 114049, 114053, 114055, 114069 (b) Observed several bags of flour stored on the floor throughout facility. Store all bags of flour at least 6 inches off the floor. Correct immediately.

33: 114115(b), 114257 Observed unclean ventilation filter. Clean/correct in 15 days.

35: 114130, 114130.1, 114130.2, 114130.3, 114130.4, 114130.5, 114130.6, 114132, 114133, 114137, 114139, 114153, 114163, 114165, 114169, 114175, 114177, 114180 Household microwave (Panasonic Inverter) observed used for customer food items at the front sales counter. Replace with commercial microwave within 30 days. -REPEAT VIOLATION- Observed white trash bag to cover croissant sandwich inside front register 2-door refrigerator. Discontinue using trash bag to cover food items and use approved food grade covers to cover food items. Correct immediately.

36: Observed multi-use utensils stored in bucket using unapproved method. Provided education to store utensils at below 41F (e.g. submerged in an ice bath), above 135F (e.g. inside a steam table insert) or in a sufficiently acidic solution (pH<4.6). Correct immediately.

38: 114149, 114149.1, 114149.2, 114149.3, 114252, 114252.1 Light out inside hood ventilation system. Replace light bulb. Correct within 7 days. -REPEAT VIOLATION-

40: 114135, 114185, 114185.1, 1141185.2, 114185.3, 114185.3, 114185.4, 114185.5 Observed no sanitizer bucket available. Setup sanitizer bucket at beginning of operation and maintain 100ppm (chlorine) at all times. Correct immediately.
