1c: 113948 All employees' food handler cards were not available to review. Correct within 30 days.

4: 113977, 114256 (a) Employees' drinks stored on top of reach-in coolers, storage racks throughout kitchen. Provide cup with tight fitting lid and straw and store all the employee drinks to the designated area (below or away from food or food related products) today. (Repeat) Observed employees eating in kitchen food prep area. Cease practice immediately and eat only in non-food storage/prep areas (ie: the dining room). Corrected; education provided.

5a: 113952, 113953.3, 113953.4 Observed employee touched mouth with glove hand. Inspector directed employee to wash hands. Observed employee washed hands with soap and hot water, but did not dry hands with paper towels. Corrected.

6: 113953, 113953.1, 113953.2, 114067(f), 114143(e) Handwashing sink next to 3-comp ware sink was partially blocked by a large pot and lacked paper towels. Correct immediately. NOTE: A fully stocked handwashing sink was accessible about 15-feet away. (Repeat Violation) Paper towels stored outside of dispenser in small prep room near restrooms. Correct immediately.

7: 113996, 113998, 114037 Major Violation: A small plate of raw spicy pork belly stored on top of cold top insert and pork belly measured at 55F. Per employee, he did not know how long the pork belly has been sitting there. Food discarded. Corrected. A small plate of tofu skin filled shrimp pate observed inside the reach-in cooler and food measured at 51-52F. Per employee, he just relocating the food back to the refrigerator. Corrected. Deep fried tofu measured at 56F at cold top of the reach-in cooler. Food filled a little over the insert line. Per manager, employee might leave the food out at the push cart for too long. Manager voluntarily discarded food items. Corrected. A pan of raw shell eggs (20+) observed on ice at the self-service table. Raw shell eggs measured at 55-66F for the top 2 layers. Manager stated that raw shell eggs have been out for 1 hour. Manager voluntarily discarded food items. Corrected. Minor Violation: The following potentially hazardous foods were found out of proper holding temperature: - lobster balls at 47F - cold top of the reach-in cooler - siumai at 46F - cold top of the reach-in cooler - mussel at 48F inside reach-in cooler - ginger in oil at 48F - self service table - kelp noodle with mayo at 50F - self service table - pig ear at 50F - self service table - spicy fish paste at 50F - self service table - cut honeydew at 49F on ice - self service table - crab meat stick at 50F - cold top of the reach-in cooler inside side prep room - deep fried tofu at 50F - cold top of the reach-in cooler inside side prep room Maintain all potentially hazardous food at or below 41F at all times. Correct immediately.

14a: 113984(d), 114113, 114115, 114117, 114121(a), 114125(b), 114141, 114180(a-c) Old food debris build-up was observed on meat slicer blade and housing. Clean and sanitize within 24 hours. Observed old food residue on clean bowls/plates that located at the storage rack right next to the meat slicer. Contaminated bowls/plates placed in dishwasher for rewash and re-sanitize. Corrected.

14b: 114097, 114099.1, 114099.2, 114099.4, 114099.6, 114099.7, 114101, 114105, 114109, 114111, 114185.3 (a-c) MAJOR VIOLATION - Low temperature dish machine in kitchen measured 0 ppm chlorine sanitizer. Repair technician was called at the time of inspection. Repair within 24 hours. NOTE: Manual ware sink was set up at the time of inspection and measured 300 ppm quaternary ammonia sanitizer.

16: 114039, 114039.1, 114039.3, 114039.4 Shellfish tags are stored in a mixed bag above prep sink. Organize in chronological order within 7 days. Upon arrival, oysters were being washed inside food prep sink. When they were observed again in the walk-in cooler, they were without the shellfish tags. Corrected. NOTE: Tags must be kept with batch until the last one is used.

21: 113953(c), 114189, 114192, 114192.1, 114195 Measured hot water from sensor (premixed) faucet in women's restroom as high as 82F. Maintain hot water at premixed faucet between 100F-108F. Correct within 24 hours

23: 113903, 114259, 114259.1, 114259.4, 114259.5 Observed several house flies flying in food prep areas. Eliminate immediately by using approved methods only. Observed multiple fly strips with many dead flies stored within food prep area. These are only approved for use in janitorial areas, or in areas where no food is being stored or prepped. Remove today. Eliminate fly issue immediately. If you have fly issue, please contact your pest control company.

26: 114018, 114020 A box of frozen raw beef short ribs observed thawing on the counter. Bags of frozen sausage, beef tendons and etc. observed thawing in blue push carts throughout kitchen. Proper thawing education provided. Corrected by relocating all the food items back to the refrigerator.

27: 113984, 113986, 114060, 114069, 114077, 114089.1 (c), 114143 (c-d), 114073 Raw shrimps stored on top of cooked crawfish inside reach-in cooler. Observed raw oysters and raw shrimp stored above produce in walk-in cooler. Raw beef was stored above raw oysters inside walk-in freezer. Rearrange all the potentially hazardous food per proper storage stickers today. (Repeat Violation) Sneeze guard at the self -service table was not extending long enough to protect the first line of food items. Correct within 7 days. Open bulk containers observed at storage racks throughout kitchen. Correct immediately.

28: 113992 Observed employee cutting green onion before it had been washed. All produce must be washed prior to its preparation.

30a: 114047, 114049, 114053, 114055, 114069 (b) Observed boxes of food items stored directly on floor in walk-in freezer. Store at least 6 inches off the floor today. (Repeat Violation)

33: 114115(b), 114257 Observed excess water on floor in warewashing area. Keep floors dry to prevent attracting vermin and deteriorating the floor tile grout. Correct within 24 hours and maintain. Excess food debris observed inside 3-comp sink drains, preventing them from sealing. Corrected.

34: 113984(e), 114067(f,g), 114099, 114099.3, 114099.5, 114101(a), 114101.1, 114101.2, 114103, 114107, 114125 Lacking chlorine sanitizer test strips. Correct within 24 hours. (Repeat Violation)

35: 114130, 114130.1, 114130.2, 114130.3, 114130.4, 114130.5, 114130.6, 114132, 114133, 114137, 114139, 114153, 114163, 114165, 114169, 114175, 114177, 114180 Observed household microwave in kitchen and black household chest freezer in bar. Replace with ANSI/NSF approved commercial grade units within 6 months. Raw wood board stored below bulk ingredient storage used to support the weight of items on shelf. Seal or paint with 3 coats of white semi-gloss paint. All surfaces must be smooth, durable, easily cleanable and nonabsorbent. Observed heavy leak below kitchen dish machine. Repair within 7 days. Goose neck sprayer at dish machine is nonfunctional. Operator has installed a hose without any anti-siphon protection to a hot water line instead. Repair goose neck sprayer and remove hose from waterline within 7 days. Observed peeling duct tape used to label racks inside 2-door upright reach-in cooler next to ice machine. Remove within 7 days and used approved materials only.

36: 114074, 114081, 114119, 114161, 114172, 114178, 114179 Bowls used as scoops for noodle in bulk containers at storage racks. Use approved scoop or tong with handle. (Repeat Violation) Tong handle was in contact with raw meat and stored inside food container - cold top of the meat reach-in cooler. Correct immediately.

41: 114171, 114190, 114192, 114193, 114193.1, 114199, 114201, 114269 Hoses attached to the 2-way hose valve (Y-splitter), which is attached to mop sink faucet, lacking anti-siphon devices. (eg hose bibb vacuum breaker - on each end of the valves). Correct within 5 days. (Repeat Violation) Ice machine drain pipe is draining directly in floor sink. Provide a minimum 1" air gap within 7 days.

44d: 114256, 114256.1. Observed a couple of cell phones stored on top of shelves in food prep areas. Relocate immediately.

44e: 114259, 114259.2, 114259.3 Front door was propped open. Correct immediately.

45: 114143 (d), 114266, 114268, 114271, 114272 Observed damaged floor tile and missing grout in warewashing area. Repair within 30 days.

52: Note: Food items that at convey belt were under Time as Public Health Control. Employees would use black plates/utensils for the 1st TPHC round then use brown plates/utensils for the 2nd TPHC round in the convey belt. 40+ inches open window observed at convey belt for customers to grab self-service food items.
