5a: 113952, 113953.3, 113953.4 Observed employee washing hands without using hand soap and wiping hands with paper towels before handling food items. Stop employee and directed employee to wash hands with soap and dry hands with paper towels. Corrected on site.

13: 113967, 113976, 113980, 113988, 113990, 114035, 114039.2, 114167, 114254.3 MAJOR VIOLATION: Observed employee grabbing raw meat with bare hands and went straight to grabbing cooked chicken without washing hands and putting on gloves. Employee voluntarily discarded tray of cooked chicken. Discontinue practice using bare hands touching food items, and wash hands before prepping food items after handling raw meat. Corrected on site.

14a: 113984(d), 114113, 114115, 114117, 114121(a), 114125(b), 114141, 114180(a-c) Observed black/white residue inside ice machine and on ice scoop. Wash, rinse, sanitize, and air dry inside ice machine and ice scoop. Correct in 24 hours.

26: 114018, 114020 Thawing raw meat on prep counter. Discontinue practice and use approved method to thaw meat. Correct immediately.

Cover all food items at all times when not in use. Correct immediately. Observed raw meat stored above wonton wrap inside 2-door upright freezer. Store raw meat below wonton wrap. Correct immediately.

30b: 114051 Observed several unlabeled food containers on shelf above prep counter. Label all food containers with food common name. Correct immediately.

The label to bags of bread and chili oil must include the following: 1. Common name of food 2. Name and address of business, manufacturer or distributor 3. List of ingredients 4. Net weight Correct within 3 days. Correct immediately.

33: 114115(b), 114257 Observed food debris on the floor below cooking equipment. Clean/correct in 7 days. Observed unclean ventilation filter. Clean/correct in 15 days.

Provide sanitizer bucket with chlorine/quaternary ammonia sanitizer and maintain sanitizer level at 100ppm/200ppm at all times. Correct immediately. Observed 0ppm sanitizer residue on multiple wiping cloth on multiple prep counters. Store wiping cloth in sanitizer bucket when not in use. Correct immediately.
