Observed unclean food contact surfaces. Observed several food container lids soiled.
Corrective Actions: Equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean within 48 hours.
14: 14.5 - 25 TAC §228.38(d)
Observed food handler hands and arms not being washed when required. Observed no handwashing by food employee between tasks.
Corrective Actions: Food employees shall clean their hands and exposed portions of their arms as specified under §228.38(b) (Proper Handwashing Procedure) immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single- service and single-use articles and: after touching bare human body parts other than clean hands and clean, exposed portions of arms; after using the toilet room; after caring for or handling service animals or aquatic animals; after coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking; after handling soiled equipment or utensils; during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; when switching between working with raw food and working with ready-to-eat food; before donning gloves to initiate a task that involves working with food; and after engaging in other activities that contaminate the hands. Reeducated food employees on when to wash hands.
21: 21.9 - 25 TAC §228.33(a)
Failure to provide a supervisor or manager with authority over food operations that has received a certified food manager accreditation. Observed no certified food manager on duty at time of visit. This is a repeat violation. Provide at least one certified food manager on duty within 10 business days.
Corrective Actions: Shall provide at least one employee with management responsibility who has received a certified food protection manager accreditation.

Failure of food employees to complete an accredited food handler training course. This is a repeat violation. Ensure all food employees obtain an approved food handler card within 10 business days.
Corrective Actions: All food employees shall successfully complete an accredited food handler training course, within 60 days of employment.
31: 31.11 - 25 TAC §228.175(b)
Soap dispenser with soap not provided at handsink. Observed soap missing at hand sink.
Corrective Actions: Shall provide dispenser with liquid hand soap at all handsinks at all times. Employee provided soap immediately.
35: 35.1 - 25 TAC §228.42(a)(1)
Observed employee eating food/chewing gum/drinking beverages (in unapproved manner)/ using tobacco in kitchen area or food prep area. Observed cigarette butts in the kitchen. Employee said he smokes in the kitchen sometimes.
Corrective Actions: Employees shall eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils, and linens; unwrapped single-service and single-use articles; or other items needing protection cannot result. Shall train employees to use designated eating, drinking, or tobacco use areas. Refrain from smoking in the kitchen.

Observed employees not wearing hair/beard restraints.
Corrective Actions: Shall wear effective hair/beard restraints. Correct by end of day today.
36: 36.3 - 25 TAC §228.68(d)(2)(A)
Observed cloths in use for wiping counters and other equipment surfaces not stored in chemical sanitizer solution between uses. Observed wiping towels all over the place instead of in chemical sanitizer solution between uses.
Corrective Actions: Shall store wiping cloths in chemical sanitizer solution at a concentration specified under 25 TAC §228.11 between uses. Correct violation within 24 hours.
41: 41.6 - 25 TAC §228.66(b)
Observed facility using a non-original container of food and/or food ingredients, that are not readily and unmistakably recognized, that has not be labeled with the common name of the food item. Observed bulk food in generic storage bins not labeled with the common name of the food.
Corrective Actions: Except for containers holding food that can be readily and unmistakably recognized such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be identified with the common name of the food. Label.
42: 42.2 - 25 TAC §228.113(3)
Observed nonfood-contact surface of equipment has accumulated dust, dirt, food residue, and other debris. Observed the exterior of prep cooler lids with thick layer of flour. Observed pizza cooler doors soiled.
Corrective Actions: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Clean within 72 hours.
45: 45.14 - 25 TAC §228.186(b)
Observed that facility is not being cleaned as often as necessary to keep it clean and/or facility is being cleaned during periods when a large amount of food is exposed. Observed floors and floor sink under 3 comp sink soiled.
Corrective Actions: The physical facilities shall be cleaned as often as necessary to keep them clean. Except for cleaning that is necessary due to a spill or other accident cleaning shall be done during periods when the least amount of food is exposed such as after closing. Shall clean when the least amount of food is exposed and as often as necessary to maintain facility clean. Clean within 5 business days.

Observed facility has not posted their City of Plano Food Establishment Permit on site in a location that is conspicuous to consumers.
Corrective Actions: Upon acceptance of the permit issued by the regulatory authority, the permit holder in order to retain the permit shall post the permit in a location in the food establishment that is conspicuous to consumers. Shall post permit in conspicuous location.
