6-301.14 VIOLATION OF SECTION 6-301.14 A handwashing sign is not posted at all handwashing sinks used by food employees, specifically: **there is no handwashing sign posted in the female restroom used by food employees.
Corrective Actions: REQUIRED CORRECTION: A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees.
3-302.11(A)(1,2) VIOLATION OF SECTION 3-302.11(A)(1,2) *PRIORITY* Raw or ready-to eat food is not properly protected from cross contamination, specifically: ** In the reach-in freezer there was a package of raw chicken was placed on top of a loosely wrapped raw hamburger beef and a ziploc bag of ready-to-eat bacon bits.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Foods shall be protected from cross contamination during storage, preparation, holding, and display by separating raw animal foods and unwashed produce from ready-to-eat foods, ready-to-eat raw animal foods (fish for sushi), fruits and vegetables, and by separating different types of raw animal foods from each other. If stored vertically, raw animal foods should be stored in ascending order of cooking temperature, with the highest required cooking temperature at the lowest level.
Action Taken: We discussed the importance of verticle storage of ready-to-eat and raw food. The operator is shifting the food into different bins to create separation.
3-501.16 VIOLATION OF SECTION 3-501.16 *PRIORITY* Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: **Lentil soup on the steamer had a temperature reading of 124F.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Potentially hazardous foods shall be maintained at 135°F or above (except roast beef may be held at 130°F (54°C) or above) or at 41°F or less.
Action Taken: the PIC turned up the steamer table and reheated the soup to 165F +.
4-204.112(A)(B)(D) VIOLATION OF SECTION 4-204.112(A)(B)(D) Temperature measuring devices are not provided, or are not easily readable, properly located, or accurately scaled in hot or cold food storage units, specifically: no thermometers observed in the two reach-in refrigerators.
Corrective Actions: REQUIRED CORRECTION: An accurate, easily readable temperature measuring device shall be located in the warmest part of mechanically refrigerated units and in the coolest part of hot food storage units.
3-305.11 VIOLATION OF SECTION 3-305.11 Food is not stored to prevent contamination, specifically: **bag of flour sitting directly on the floor in the dry storage room.
Corrective Actions: REQUIRED CORRECTION: Food shall be protected from contamination by being stored: 1) In a clean, dry location; 2) Where it is not exposed to splash, dust or other contamination; and 3) At least 6 inches above the floor.
4-501.11 VIOLATION OF SECTION 4-501.11 Equipment is not maintained in good repair or according to manufacturer's recommendations or cutting parts of can openers are not kept sharp, specifically: **The vent cover/screen to the hood vent is missing.
Corrective Actions: REQUIRED CORRECTION: All equipment and components shall be in good repair and maintained and adjusted in accordance with manufacturer's specifications. Maintain the cutting or piercing parts of can openers in a sharp condition.
Action Required: Ensure the hood vent is in good repair and operating per the manufacturer's specifications. .
4-602.13 VIOLATION OF SECTION 4-602.13 Nonfood-contact surfaces are not cleaned at an adequate frequency, specifically: The hood vent is covered in grease and causing oil drips around the frame due to a missing screen and significant grease buildup inside.
Corrective Actions: REQUIRED CORRECTION: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
Action Required: A properly trained person should inspect the hood vent regularly. Minimum cleaning frequency: Hoods, grease-removal devices, fans, ducts and other appurtenances should be cleaned at intervals necessary to prevent the accumulation of grease
5-501.17 VIOLATION OF SECTION 5-501.17 A covered receptacle is not provided in a restroom used by females, specifically: **There is only one waste bin in the restroom dedicated to females. No covered receptacles are provided for sanitary napkins.
Corrective Actions: REQUIRED CORRECTION: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
