Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Food employees must thoroughly wash their hands and exposed portions of their arms: 1) before beginning or returning to work; 2) after touching body parts other than clean hands and clean, exposed portions of arms; 3) after using the toilet room; 4) after caring for, or handling service or aquatic animals; 5) after coughing, sneezing, or using a handkerchief or disposable tissues; 6) after using tobacco, eating or drinking from inappropriate containers; 7) after handling soiled equipment or utensils; 8) immediately before engaging in food preparation in the food preparation area; 9) during food preparation, as often as necessary to remove soil and contamination and to prevent cross-contamination when changing tasks; 10) when switching between working with raw foods and working with ready-to-eat foods; 11) before donning gloves for working with food; or 12) after engaging in other activities that contaminate the hands. EMPLOYEES MUST WASH THEIR HANDS AFTER CONTAMINATION AND BEFORE RETURNING TO PREPARING FOOD AND BEVERAGES. Action taken: DISCUSSED WHEN TO WASH WITH THE MANAGER. MANAGER WILL REVIEW WITH EMPLOYEES THE NEED FOR HANDWASHING AFTER CONTAMINATION. VIOLATION CORRECTED.

3-501.16 VIOLATION OF SECTION 3-501.16 *PRIORITY* Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: INTERNAL TEMPERATURE OF FOODS STORED IN THREE REFRIGERATION UNITS IN THE FACILITY ARE ABOVE 41 DEGREES F, SPECIFICALLY, IN THE PREP TABLE AT THE FRONT COUNTER, SLICED TOMATOES @ 52 DEGREES F, SHREDDED CHEESE @ 51 DEGREES F, PAR-COOKED CHICKEN @ 46 DEGREES F; IN THE TRUE 2-DOOR REFRIGERATION UNIT IN THE DRY GOODS STORAGE AREA: SLICED CHEESE @ 52 DEGREES F; AND IN THE TRUE SINGLE-DOOR REFRIGERATION UNIT IN THE DRY GOODS STORAGE AREA: YUMI BLUEBERRY YOGURT @ 57 DEGREES F.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Potentially hazardous foods shall be maintained at 135°F or above (except roast beef may be held at 130°F (54°C) or above) or at 41°F or less. MAINTAIN INTERNAL TEMPERATURES OF POTENTIALLY HAZARDOUS FOODS AT COLD HOLDING TEMPERATURES OF 41 DEGREES F OR BELOW. Action taken: THE OPERATOR BEGAN WORKING TO REMOVE AND DISCARD FOODS THAT ARE OUT OF TEMPERATURE CONTROL. HAVE THE REFRIGERATION UNITS REPAIRED OR REPLACED. DISCUSSED CAPACITY OF REMAINING REFRIGERATION. THE OPERATOR STATED THEY WILL NEED TO REDUCE THE MENU AVAILABLE TO PASTRIES AND COFFEE BEVERAGES. A RE-INSPECTION WILL OCCUR WITHIN 14 DAYS TO CONFIRM THE PROPER OPERATION OF THE REFRIGERATION UNITS.

7-201.11 VIOLATION OF SECTION 7-201.11 *PRIORITY* Poisonous or toxic materials are not properly separated or located, specifically: CANS OF RAID AND FLY SPRAY ARE STORED UNDER THE COUNTER ADJACENT TO FOOD STORAGE AT THE FRONT COUNTER SERVICE AREA.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by spacing or partitioning. Poisonous or toxic materials shall be located in an area that is not above food, equipment, utensils, linens or single-service or single-use articles. SEPARATE POISONOUS OR TOXIC MATERIALS AND LOCATE SO THEY DO NOT CONTAMINATE FOOD, EQUIPMENT, UTENSILS, OR SINGLE-SERVICE ITEMS. Action taken: THE OPERATOR REMOVED AND RELOCATED THE CANS OF RAID AND FLY SPRAY DURING THE INSPECTION. A DETERMINATION SHOULD BE MADE IF THESE CHEMICALS ARE NEEDED AT THE LOCATION. VIOLATION CORRECTED.

7-204.11 VIOLATION OF SECTION 7-204.11 *PRIORITY* Chemical sanitizers are not approved, specifically: THE SANITIZER TOWEL BUCKET AT THE FRONT COUNTER TESTS AT 200 PPM OR GREATER.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Chemical sanitizers and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 CFR 180.940. PREPARE CHLORINE BLEACH SOLUTIONS WITH A CONCENTRATION BETWEEN 50-100 PPM. Action taken: THE MANAGER DILUTED THE CHLORINE BLEACH SOLUTION TO 100 PPM. VIOLATION CORRECTED.

6-501.111(A)(B)(D) VIOLATION OF SECTION 6-501.111(A)(B)(D) The presence of insects, rodents, and other pests is not controlled to minimize their presence, specifically: THERE ARE NUMEROUS FLIES IN PREPARATION AREAS IN THE FACILITY, INCLUDING THE FRONT COUNTER AND BACK BAKERY.
Corrective Actions: REQUIRED CORRECTION: The premises shall be maintained free of insects, rodents, and other pests. Their presence shall be eliminated by routinely inspecting incoming shipments of food and supplies, routinely inspecting the premises for evidence of pests, and eliminating harborage conditions. MAINTAIN THE PREMISES FREE OF INSECT AND RODENT ACTIVITY. Action taken: DISCUSSED THE NEED FOR PEST CONTROL IN THE FACILITY. THE OPERATOR MAINTAINS DOORS OPEN FOR VENTILATION. INSPECTOR PROVIDED PEST CONTROL FACT SHEET AND ADDITIONAL RESOURCE. INSPECTOR WILL CHECK THE FACILITY AT RE-INSPECTION FOR INSECT ACTIVITY.

5-205.15(B) VIOLATION OF SECTION 5-205.15(B) The plumbing system is not maintained in good repair, specifically: THE URINAL IN THE MEN'S RESTROOM IS OUT OF ORDER. THIS HAS BEEN NOTED ON PREVIOUS INSPECTION REPORTS.

6-501.12 VIOLATION OF SECTION 6-501.12 The establishment is not cleaned as often as necessary or at a time when the least amount of food is exposed, specifically: AREAS OF CABINETRY STORAGE IN THE FRONT SERVICE AREA HAVE A BUILD-UP OF SOILS AND DEBRIS.
Corrective Actions: REQUIRED CORRECTION: The physical facilities shall be cleaned as often as necessary to keep them clean. Cleaning shall be done during periods when the least amount of food is exposed, such as after closing. CLEAN AS OFTEN AS NECESSARY TO PREVENT A BUILD-UP OF SOILS AND DEBRIS.

6-101.11 VIOLATION OF SECTION 6-101.11 Floors, walls and ceilings are not smooth, durable, easily cleanable or nonabsorbent, or floors are improperly carpeted, specifically: THERE ARE SECTIONS OF WALL THAT HAVE HOLES AND ARE NO LONGER SMOOTH, DURABLE, OR EASILY CLEANABLE, INCLUDING THE WALL ADJACENT TO THE SERVICE SINK IN THE WAREWASHING AREA, AND THE WALLS IN THE BAKERY, ESPECIALLY AT THE FRONT DOOR TO THE BAKERY.
Corrective Actions: REQUIRED CORRECTION: Smooth, nonabsorbent, easily cleanable, and durable floors, wall and ceiling surfaces shall be provided. Provide nonabsorbent floor, wall and ceiling surfaces for food preparation areas, walk-in refrigerators, warewashing areas, toilet rooms, handwashing areas, storage areas, janitorial areas, laundry areas, interior garbage and refuse storage rooms, areas subject to flushing or spray cleaning methods, and other areas subject to moisture. Corrective Actions: REQUIRED CORRECTION: The premises shall be free of litter and unnecessary articles which are not pertinent to the current operation of the food establishment. REMOVE THE UNNECESSARY ARTICLES FROM THE FACILITY TO PREVENT HARBORAGE CONDITIONS.
