3-501.16 VIOLATION OF SECTION 3-501.16 *PRIORITY* Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: RIGHT SIDE PREP COOLER TOP (LEFT OPEN FOR EXTENDED PERIOD) NEXT TO GRILL HAS RAW BURGER PATTIES AND PRECOOKED SAUSAGE PATTIES MEASURE 50-53 F.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Potentially hazardous foods shall be maintained at 135°F or above (except roast beef may be held at 130°F (54°C) or above) or at 41°F or less. ALL POTENTIALLY HAZARDOUS FOODS HELD LONGER THAN 4 HRS ON PREP TABLE TOP ARE DISCARDED. KEEP TOP SHUT TO PREVENT WARMING OF FOOD FROM HIGH TEMPERATURE AMBIENT AIR FROM GRILL.

5-501.116 VIOLATION OF SECTION 5-501.116 Receptacles and waste handling units are not cleaned in a way that prevents contamination or at an appropriate frequency, specifically: COOKING OIL RECYCLING BIN IS COVERED IN OIL AND IS POOLING ON TOP SURFACE OF BIN.
Corrective Actions: REQUIRED CORRECTION: Receptacles and waste handling units for refuse, recyclables or returnables shall be cleaned in a way that does not contaminate food, equipment, utensils, linens, or single-service or single-use articles. Soiled receptacles and waste handling units for refuse, recyclables and returnables shall be cleaned at a frequency necessary to prevent them from developing a build-up of soil or becoming attractants for insects and rodents.

5-501.115 VIOLATION OF SECTION 5-501.115 The storage area or enclosure for refuse, recyclables or returnables is not maintained free of unnecessary items, specifically: REFUSE AND RECYCLING AREA HAS ACCUMULATION OF LOOSE TRASH BEHIND BINS.
Corrective Actions: REQUIRED CORRECTION: A storage area or enclosure for refuse, recyclables or returnables shall be maintained clean and free of unnecessary items and litter. CLEAN AND MAINTAIN TO PREVENT PEST HARBORAGE.

6-501.14 VIOLATION OF SECTION 6-501.14 Intake or exhaust air ducts are not maintained properly, or outside venting creates a hazard or nuisance, specifically: INTERIOR AND BAFFLES OF EXHAUST HOOD OVER COOK LINE HAVE HEAVY ACCUMULATION OF GREASE.
Corrective Actions: REQUIRED CORRECTION: Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard, nuisance or unlawful discharge. CLEAN AND MAINTAIN.
