Each handwashing sink or group of adjacent handwashing sinks shall be provided with a supply of individual disposable towels, a continuous towel system that supplies the user with a clean towel, or a heated-air hand drying device. 5-205.11 VIOLATION OF SECTION 5-205.11 *PRIORITY FOUNDATION* A handwashing sink is not accessible for employee use at all times, is used for purposes other than handwashing or is not operated properly, specifically: KITCHEN HAND-WASH SINK OUT OF ORDER. STAFF WASHING HANDS AT FRONT HAND-WASH SINK WITH DOOR CLOSED BETWEEN THOSE TWO AREAS.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Handwashing sinks shall be accessible at all times for employee use and not used for other purposes. An automatic handwashing sink shall be used in accordance with manufacturer's instructions. TEMPORARY CORRECTION: PROPPED OPEN DOOR BETWEEN KITCHEN AND FRONT HAND-WASH SINK. REPAIR KITCHEN HAND SINK IMMEDIATELY. I WILL RECHECK THIS AROUND TWO WEEKS FROM TODAY.

4-501.114(E) VIOLATION OF SECTION 4-501.114(E) *PRIORITY* A chemical sanitizer in use other than chlorine, iodine, or a quaternary ammonium compound is not applied properly, specifically: FRONT SANITIZER BUCKETS USED FOR TABLES AND. FOOD CONTACT SURFACES NOT SHOWING POSITIVE FOR ANY OF THE TEST KITS USED.
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. If a chemical sanitizer other than chlorine, iodine, or a quaternary ammonium compound is used, it shall be applied in accordance with the EPA-registered label use instructions. UNTIL CORRECTED. USING THE OTHER PEROXIDE ECOLAB SANITIZER THAT HAS TO BE HAND-DILUTED TO BETWEEN 272-700 PPM. WILL FIGURE OUT WHAT TEST STRIPS NEEDED FOR THE FRONT TYPE OF SANITIZER AND THEN USE AT THE PROPER LEVEL. I WILL RECHECK THIS AROUND TWO WEEKS FROM TODAY.

7-202.12(A)(2)(3)(B) VIOLATION OF SECTION 7-202.12(A)(2)(3)(B) *PRIORITY* Poisonous or toxic materials are improperly used or applied, specifically: SINK AND SURFACE CLEANER SANITZER FOUND AT COOKLINE AT 848 PPM WHICH IS ABOVE THE ALLOWABLE LEVEL. IT IS DISPENSING AUTOMATICALLY AT THIS LEVEL
Corrective Actions: REQUIRED CORRECTION: CORRECT IMMEDIATELY. Poisonous or toxic materials shall be used according to label directions and applicable laws. Poisonous or toxic materials shall be applied so that a hazard to employees or other persons is not constituted and so that contamination of food or other items will not result. HAND-DILUTED THE BUCKETS TO 272 PPM. KEEP BETWEEN 272-700 PPM. WILL HAND-DILUTE UNTIL THIS IS CORRECTED. I WILL RECHECK THIS AROUND TWO WEEKS FROM TODAY TO SEE THAT YOUR SANITIZERS ARE AUTOMATICALLY DISPENSING AT PROPER LEVELS.

3-304.12 VIOLATION OF SECTION 3-304.12 In-use dispensing utensils are not properly stored, specifically: IN USE KNIVES KEPT IN DRY WELL AT COOKLINE.
Corrective Actions: REQUIRED CORRECTION: In-use food preparation or dispensing utensils shall be stored: 1) in the food with the handles above the top of the food; 2) on a clean portion of the food preparation table or cooking equipment if they are cleaned and sanitized as required; 3) in running water of sufficient velocity to flush particulates to the drain; 4) in a clean, protected location if the utensils are used with food that is not potentially hazardous; or 5) in a container of water at a temperature of 135°F or more or 41°F or less or if the container is cleaned as required.
