2-2D: .07(3)(a)2 handwashing sinks in the kitchen do not have soap. Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder, or bar soap. (Pf)
Corrective Actions: Soap was placed at the handwashing sinks.
4-2B: .05(7)(a)1Observed black mold like substance in the interior of the ice machine. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)
8-2B: .07(6)(c)Observed Quat sanitizer bucket stored directly behind the cutting board on the prep table. Observed chemical spray bottle stored over salt and pepper shakers on the counter next to the front handwashing sink. Observed Quat sanitizer spray bottle and large container of hand sanitizer stored next to single use straws at the bar. Observed 2 bottles of grill cleaner stored next to clean pans and oven racks in the kitchen. Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles. (P)
Corrective Actions: All chemicals were relocated to a designated spot for chemicals.
9-2: .04(6)(k)Facility placing canned baked beans and smoked pork in reduced oxygen packages and facility does not have a HACCP or variance for this process that has been approved by Georgia's state health department. Facility has held TCS foods in ROP longer than 48 hours (canned baked beans marked 2/17-2/22 and pulled pork marked 2/18-2/25). A food service establishment that packages time/temperature control for safety food using a reduced oxygen packaging method shall implement a HACCP plan that contains the information specified under DPH Rule 511-5-14-.02(6)(b) and (d) and that meets all other requirements of subsection .04(6)(k)1. **3rd CONSECUTIVE REPEAT VIOLATION ON A ROUTINE INSPECTION**
Corrective Actions: Foods discarded. Discussed with person in charge that facility must apply for a variance or HACCP with Georgia's Public Health Department if they want to store foods in ROP for more than 48 hours.
17D: .07(4)(b)Observed multiple employee items stored in unapproved areas (employee drink in a single use cup with no lid or straw stored over food in the raw meat walk in cooler, 2 employee jackets stored with single use items, employee air pod stored over the sauce prep cooler). Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. (C)
Corrective Actions: Drink and air pod were discarded. Employee jackets moved to employee designated storage bin.

NOTE: All cold holding and hot holding temperatures in compliance, unless noted.
NOTE: Facility uses flavored syrups for drinks.
NOTE: Ensure non food contact surfaces of equipment, floors, and walls are cleaned more frequently.
NOTE: Per person in charge, the ceiling tiles have been approved by corporate and the mall for repair. Per person in charge, facility is waiting on ceiling installation.
NOTE: An informal follow up will occur on 2/29/24.
**Food service permit SUSPENDED due to 3 consecutive REPEAT violations on a routine inspection for not following the criteria of reduced oxygen packaging when facility does not have an approved variance or HACCP plan [9-2 Violation of Code: .04(6)(k)]. On site training provided. Food service permit REINSTATED.**
**4 consecutive REPEAT violations on a routine inspection may result in food service permit SUSPENSION.**
