THIRD CONSECUTIVE VIOLATION- PERMIT SUSPENDED** Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food. 14B: .05(10)(e) 1,2,4Observed multiple containers that were stored as clean, but stacked still wet. Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted. (C)

17C: .07(5)(a),(b)1,2,3Observed vent hood and wall behind the grill with grease accumulation on them. **SECOND CONSECUTIVE VIOLATION** All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. 17D: .07(3)(f)Observed two out of four light bulbs inside the vent hood that are burned out. The cooking area does not have adequate lighting. The light intensity shall be at least 50 candles at the surface where a food service employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.
